Hyperextension
Drop arm test application
Watch the patient make slow snow angel arms. Then, to intensify, apply downward pressure to both arms.
Upper extremity test
Observes standing active shoulder abduction.
What do you do if the drop arm test is negative?
Move on to the empty and full can tests
Positive sign of drop arm test
Pain, apprehension or weakness
Mechanics of drop arm test
Downward pressure causes deltoid to contract, squeezing the subacromial space.
Drop arm test tests for…
Subacromial impingement, either by the suprascapular tendon or subacromial bursa
Prolonged injury of the subacromial space can lead to what?
Adhesive capsulitis
Empty can test application
Patient holds arms at 90 degrees, then rolls hand forward to “empty the can.” Apply downward pressure.
Structures tested in the full and empty can tests
Subacromial impingement.
How is adhesive capsulitis treated with massage?
Generalized massage on the shoulder muscles to improve mobility.
Structure tested with Speed’s test
The long head of the biceps tendon. This passes through the bicipital groove.
Speed’s test application
View active GH flexion with slow extension. If this is not painful, place the practitioner thumb on the tendon. Press to a 7 on a pain scale and hold this pressure for 7 seconds.
Speed’s test positive sign
The pain gets worse or stays the same throughout 7 seconds.
Structure tested with Yergeson’s test
The capacity of the annular ligament across the bicipital groove to hold the tendon in the groove.
Yergeson’s test application
Have the bicep in a contracted position. Medially and laterally rotate the arm. Practitioner feels to see if they feel the tendon snapping. To intensify, add load to the hand.
Yergeson’s test positive signs
Pain
AP glide test application
Drawer test for the GH. Hold the scap still with one hand and move the humerus with the other.
