Doctors, dentists, supplementary prescribers, all independent prescribers, practitioner nurse prescribers and EEA/Swiss Prescribers.

HCP’s outside EEA/Switzerland = not recognised as relevant in the UK

  • Satisfied it has been used before - SCR (with permission), old bottles etc.
  • Dose confirmation
  • Not for CD’s except phenobarbital

  • Name and address of owner
  • Identification and species of animal
  • Date - valid for 6 months and 28 days for CD2, 3, 4
  • Name, quantity, dose and admin instructions
  • Any necessary warnings
  • Use under cascade if necessary
  • For sch 2 and 3 - a declaration that item has been prescribed for an animal or herd under the care of a veterinarian
  • If it is repeatable - the number of times it can be repeated needs to be on there
