Three Waves of Opioid Overdose Deaths
Wave 1: oxycodone and some newer opioids prescribed; doctors misinformed and told they were not addictive; culture of marketing in pharmaceuticals; pressure to treat pain w/o knowledge of addiction
Wave 2: prescription opioids became restricted, pts turned to sources like heroin
Wave 3: fentanyl flooded the market, pts exposed w/o knowledge due to inclusion in other opioids illicitly
SBIRT
Screening, Brief Intervention, Referral to Treatment
Looking for people at risk but not yet diagnosed, most likely to develop SUD in the future
Public Stigma
Society’s negative attitudes towards a group of people
Structural Stigma
Systems-level discrimination such as cultural norms, institutional practices, healthcare policies etc.
Self-Stigma
Individuals internalize and accept negative stereotypes
Stigma against medications
FDA approved medications are thought to be “trading one addiction for another” despite effectiveness
Stigma by association
Family, friends and others distance themselves from stigmatized individual to avoid judgement or devaluation
