Acetaminophen (Tylenol)

A

•antipyretic; nonopioid analgesic
•treatment of mild pain and fever
•SE: rash, hives, *hepatotoxicity
•CI: alcohol, warfarin, hepatic impairment, concurrent use of NSAIDs

For validation:
•assess for pain and fever and liver function
•”This medication is to lower your pain level and your fever. If you experience any rash, hives, or headache please notify me”
•”contraindications include: hepatic impairment, warfarin, and alcohol”

Ampicillin (Unasyn)

A

*anti-infective
*treatment of skin infections, soft tissue infections, ear infection, GU infection, respiratory infections
*SE: rash, diarrhea
*CI: hypersensitivity to penicillins, warfarin

A

•antianginal, antihypertensive
•tx of HTN, chest pain, MI
•SE: hypotension - dizziness, fatigue, weakness
•CI: uncompensated HF, pulmonary edema, general anesthesia, digoxin

For validation:
•check the clients BP before administering
•”this medication is to lower your blood pressure, you may feel dizziness, fatigue or weakness. If you need to get out of bed for any reason please use your call light so that I can assist you”

Ceftriaxone (Rocephin)

A

•anti-infective
•tx if intraabdominal infections and septicemia
•SE: n/v/d, rash
•CI: hypersensitivity to cephalosporins, loop diuretics, NSAIDs, warfarin

For validation:
•check for allergies
•”this is your medication for your infection. You may experience n/v/d. If you experience any of those or you begin to develop a rash please let me know”
•”contraindicated for hypersensitivity to cephalosporins or penicillins, warfarin, loop diuretics”

Enoxaparin (Lovenox)

A

•anticoagulant, blood thinner
•tx of blood clots: DVT, PE; acute STEMI
•edema, dizziness
•CI: drugs that affect platelets or coagulation, warfarin, NSAIDs, thrombolytics

For validation:
•before administering, assess patient for active bleeding and check labs
•”this is your blood thinning medication. This puts you at increased risk for bleeding and you may experience dizziness. If you need to get out of bed for any reason please let me know so that I can assist you”

•antipsychotic
•tx of chronic psychotic disorders
•SE: blurred vision, dry eyes, dry mouth, constipation
•CI: lactation, antihistamines, alcohol

For validation:
•assess behavior
•”this is your medication to help balance your mood. You may experience blurred vision, dry eyes or mouth, constipation, or an upset stomach”
•”contraindicated in Parkinsonism, kidney or heart disease, antihypertensives, and alcohol”

A

•anti-diabetic, Lowers blood sugar
•tx of diabetes, high blood sugar
•SE: hypoglycemia, dizziness, nausea
•CI: hypoglycemia, beta blockers, estrogen

For validation:
•check clients blood sugar

•CI: cross sensitivity, antidiabetic agents

•tx of managing nicotine withdrawal
•SE: increased HR, headache, insomnia
•CI: severe cardiac disease, pregnancy, caffeine, insulin

Validation:
•assess patients skin, remove old patch
•”this is your nicotine patch. You may feel an increase in your heart rate, headache, or insomnia. Let me know if you need anything”

*CI: uncontrolled bleeding, active ulcer disease
