MOA for phosphodiesterase inhibitors

A
  • nitric oxide (NO)- a substance released in men when sexually aroused, it activates guanylate cyclase and increases the synthesis of cyclic guanosine monophosphate, cGMP relaxes smooth muscle in the penis allowing increased blood flow into the penis
  • NO is metabolized by PDE, so inhibition of PDE allows for a longer erection
  • drugs only work if man is sexually aroused because NO must be present

  • all PDEIs except Cialis: take 1 hr prior to sexual activity
  • drug interactions: nitrates (nitroglycerin and isosorbide) and alpha-1 blockers
  • adverse effects:
  • -headache(Viagra has highest incidence)
  • -flushing(due to vasodilation)
  • -alter color vision(Viagra makes things look blue)
