Acceptable Pt. Identifiers

A

Use two of the following: The Pt.s full name, DOB, home address, telephone #, Last 4 of SSN, or a Photo ID. The Pt. must provide this information while you compare it with the information on the requisition.

Explaining a procedure

A

Discuss the steps of the procedure, inform the pt. they will feel a slight bit of pain, tell the Pt. to say something immediately if they have severe pain or feel sick, and briefly explain the complications of venipuncture and what to do if they occur.

Urinalysis (UA)

A

an examination of Pt.s urine for a variety of factors, including its appearance, color, odor, pH, specific gravity, and the pretense or absence of many other components such as protein, glucose, and hemoglobin.

Clean Catch Urine Specimen Collection Instructions (MALE)

A

Clean from the Meatus outward in a circular fashion. If uncircumcised, retract foreskin and use same cleansing technique. Start to urinate in the toilet, then collect urine in the container until it is 1/4 to 1/2 full, being careful not to allow the penis to touch the container. The final part of the stream should be deposited in the toilet.

Clean Catch Urine Collection Instructions (FEMALE)

A

Separate the labia between the urinary meatus and clean the area with a mild antiseptic soap and water (or antiseptic towlettes) using a front-to-back technique. Separate the labia with one hand, begin to urinate in the toilet, urinate into the container until its 1/4-1/2 full. The final part of the stream should be deposited into the toilet.

Fasting urine specimen collection instructions

A

urinate before going to sleep and then collect a specimen immediately after waking in the morning. Refrigerate the specimen and take to the lab ASAP.

Stool specimen collection instructions

A

defecate either into a sterile container or a clean container and move specimen into sterile container. Avoid getting urine or toilet water in the container, wash the outside of the container if necessary, affix the label, and wash hands thorough after the collection. Return specimen to lab immediately.

Sputum specimen collection instructions

A

collect sputum first thing in the morning. Take a deep breath, cough forcefully and deeply, and expectorate into the container. For testing, a sufficient amount of sputum is generally 1 or 2 teaspoons, close the lid, clean the outside of the container, affix the label, and deliver the specimen to lab ASAP

Semen specimen collection instructions

A

Ejaculate, deposit semen into container, secure the lid, write the date and time of the collection. Protect the specimen from extreme heat and cold. It must reach the lab within 1hr of collection.
