Myeloma Flashcards
(18 cards)
myeloma =
clonal proliferation of abnormal proliferating plasma cells that are producing a monoclonal paraprotein
main Igs involved in myeloma
IgG -50%
IgA -20%
if IgM raised more likely
Waldenstrom’s macroglobulinaemia
mean age of presentation of myeloma=
60
bone destruction in myeloma leads to
- fractures of long bones, vertebral collapses
- back ache
- hypercalcaemia
- pepperpot skull
pepperpot skull =
punched out lesions of the skull = pathognomonic of myeloma
Old CRAB for myeloma
old age calcium elevated renal failure anaemia bone lytic lesions
why do you get kidney failure in myeloma
deposition of light chains in tubules
what happens to the plasma in myeloma
hyper-viscosity (ESR high)
bad prognostic sign for myeloma
beta 2 microglobulin raised
urine findings of myeloma
bence jones proteins (excretion of light chains)
blood film of myeloma
rouleaux RBCs (coated Ig) =RBCs stick together forming chains
marrow aspiration of myeloma shows
infiltration with plasma cells
diagnostic criteria for myeloma requires
1 major + 1 minor
or 3 minor
major criteria for myeloma (3)
- plasmacytoma
- 30% plasma cells in bone marrow sample
- elevated levels of M protein in urine or blood
minor criteria for myeloma (4)
- 10-30% plasma cells in bone marrow
- minor elevations in levels of M protein in blood or urine
- osteolytic lesions
- low levels of Ig in blood
myeloma curable?
no
first line myeloma treatment
thalidomide
