What are chronic conditions?
A range of chronic and complex conditions across the spectrum of illness
What are the characteristics of chronic conditions?
They are long-term, complex, gradual onset, and compromise quality of life
At what life stages do chronic conditions occur?
They occur across all life stages, but mostly in older age
What impact do chronic conditions have on individuals?
They compromise quality of life, create limitations, and cause disability.
What are some common examples of chronic conditions?
Cardiovascular disease, diabetes, osteoporosis, neurological conditions, endometriosis, inflammatory bowel diseases.
What is the purpose of the National Strategic Framework for Chronic Conditions (NSFCC)?
(overarching policy) prevention and management.
What does the NSFCC provide guidance for?
Development and implementation of policies, strategies, actions, and services.
What are three objectives of the NSFCC?
- Focus on prevention for a healthier Australia.
- Provide efficient, effective, and appropriate care for people with chronic conditions, to optimise quality of life.
