Assoc symptoms: vomit cry, irritable, fever, poor feeding/sleeping, change in bowel habit, rash
ICE & Systems enquiry
Developmental delay
What is it (motor etc or global)
Assoc symptoms
Jaundice
onset, how long, pain/distension, bowel habit, pale stools, dark urine
Other symptoms: poor feed/sleep, dec consciousness,
Medications given recently
Medical and surgical history
FH (esp for haematological)
Age of child (conversion to HbA / breakdown of HbF e.g. day 2-15)
Seizures
Onset duration etc
What happened during seizure
Any meds tried (e.g. Lorazapam)
assoc symptoms
Any triggers, has it happened before
Any predispose: infections (e.g. measles can cause)
Rest of Hx: pmh, fh, developmental, preg histo etc
