Key Factors Dilated Cardiomyopathy

A

90% of all non-ischemic caridiomyopathies

non-ischemic: chronic alcoholism (most common cause)

Others Can be idiopathic or from viral myocarditis

(Coxsackie virus most common)

All four chambers dialated

Flabby, hypocontracting heart :(

Hypertrophic Cardiomyopathy

Key points

A

Myocardial hypertrophy; Thick walled, heavy, hyper-contracting:

Often only 1 ventricle

NO ventricular dilation: Thick Vent. septum

Abnormal diastole filling (1/3 cases)

intermittent ventricular outflow obstruction

Gentetic mutation of genes that encode sarcomere

proteins

Idiopathic hypertrophic sub-aortic stenosis

A

Intfiltratrated myocardium with abnormal tissue:

impaired vent. wall motion/abnormal contraction

amyloidosis

hemochromatosis

Key points

A

Valve leaflets!!! (sometimes endocarial lining)

Atrial surface AV valves; Vent. surface semi lunar

Vegetations most common on left valves

Easy embolism
