  • B lactam rings inhibits the enzymes responsible for cross linking peptidoglycan in the bacteria cell walls
  • = Weakened cell walls so cannot maintain an osmotic gradient

  • Skin and soft tissue infections e.g. cellulitis
  • Osteomyelitis
  • Septic arthritis
  • Other infections inc. endocarditis

  • Renal failure – dose ↓ required
  • History of penicillin allergy
  • Prior flucloxacillin related hepatotoxicity

What are the side effects of penicillinase resistance penicillin?

A
  • GI upset
  • Penicillin allergy – skin rash → anaphylactic reaction
  • Liver toxicity inc. cholestasis and hepatitis
  • Central nervous system toxicity – in high doses/severe renal impairment

• Methotrexate – penicillin ↓ renal excretion = toxicity
