Q

large rotator cuff repair
phase 3 (10-14 weeks) and phase 4 (14-18 weeks) looks like

A

AAROM –> AROM
can start isometrics 14-18 weeks

*start with flex and abduction, add in ER/IR AROM weeks 12-14

Q

what does phase 5 (22-26 weeks) look like for large rotator cuff repair

A
  1. after week 22, can do more aggressive stretching of shoulder
  2. no lifting > 10 lbs
  3. no overhead lifitng
  4. no sudden pushing or lifting
  5. no painful activity

Q

last phase of large rotator cuff repair: return to sport

(weeks 26-30) will look like

A

start plyometrics
goal of 85-90% shoulder strength of other side with HHD

precautions:
no heavy/forceful lifting, sudden stuff, no painful activity

17
Q

When can you start rotator cuff strengthening with bankhart repair?

A

phase 3 (weeks 7-8)

still no lifting > 10 lbs

18
Q

When is a good time to do CKC stuff for posterior rotator cuff activation with bankhart repair?

A

phase 5: 12-16 weeks

19
Q

when should you do functional and return to work/sport stuff for bankhart repair?

A

last phase: phase 7: early return to sport
4-6 months

20
Q

What is a latarjet

A

fixing the glenoid/labrum (dislocated shoulder so much you lose bone)

attach coracoid to where the anterior glenoid/labrum should be with screws

21
Q

What is a high tibial osteotomy

A

remove slice of proximal tibia to fix knee alignment (treat like a fracture you paid for)

22
Q

What is a xenograft augmentation?

A

using an animal graft to patch torn tendon

*bc their own tissue is damaged, maybe bc of less Blood supply or a systemic condition

23
Q

What is a Bankhart repair

A

anterior labrum anchored to glenoid: 3-6 o clock
(bc of dislocation)

24
Q

like a

25
microfracture
stimulate healing around the area by poking holes into bone (filled with fibrocartilage = asphalt)
26
what is autologous chondrocyte implantation?
take self cartilage from NWB, grow it, put it into WB cartilage area (hyaline cartilage = better than asphalt, like cement) an arthroscopic procedure
27
What does Dr. A say we NEED TO KNOW for post op
1. Surgical procedure (op note) 2. reason for procedure (MOI) 3. outcome stats 4. age and co-morbidities 5. lifestyle, goals, PLOF and health literacy 6. support (family, community, transport)
28
does bone heal faster than mm
yes, fastest bc high Blood Supply
29
does mm belly heal better than insertional tendon
yes blood supply
30
does intra-capsular ligament heal better than extra-capsular ligament
NO inside capsule = bad blood supply, ex. MCL heals better than ACL
31
If cartilage, nerve or vascular is involved, what happens to healing time
INCREASE
32
If an artery tear is involved, what does this mean?
EMERGENCY compress and go to ER | blood spurting and you can die
33
Does MCL or LCL tear of elbow heal faster?
both outside capsule LCL needs Sx though MCL not always --> so MCL
34
Why would someone get a reverse TSA
OA, OA with no rotator cuff (shot), proximal humerus fracture so bad they can't fix it and have to do this instead
35
Why do surgeons like rTSA
easier to perform surgery, less complications less pain, less complications
36
What is a big downside to rTSA
no lifting over 15 lbs for rest of life less motion for rest of life
37
how long does PT take for THA/TKA
3-6 months less pain, better function
38
how long does PT rehab of rotator cuff repair take?
6 months large repair: 6-7 months sling: 6 weeks could involve biceps tenodesis!
39
What "ability" is lost with bankart repair?
proprioception (will never feel the same)
40
TOMMY JOHN: UCL recontstruction takes how long?
formal PT 9-12 months good outcome > 85% return to sport Palmaris longus or plantaris drilled in bone (painful)
