Pulmonology Flashcards
(21 cards)
Usually caused by viruses
Cough, SOB
Clear CXR
Chronic bronchitis
Obstruction + inflam + bronchial hyperreactivity
Asthma
Most effective tx for chronic asthma
ICS
GOLD A, B, C, D tx
A: SABA
B: LABA
C: LAMA +/- LABA
D: LABA + LAMA + ICS
CXR: air bronchograms
Most common cause: Streptococcus
CAP
Young people w/ nonspecific sx (cough, pharyngitis)
Cold agglutinin assay +
Mycoplasma PNA
Always associated w/ smoking
Likely to spread early, poor prognosis
Small cell lung cancer
Tx for small cell lung cancer
Chemo
Slow growing lung cancer
Better prognosis
Non-small cell lung cancer
Tx for non-small cell lung cancer
Surgery
“Coin lesions,” sharply circumscribes, <3 cm
Tx?
Solitary pulm nodule
Surgery (40% malignant)
Non-caseating granulomas
Sarcoidosis
Tx for sarcoidosis
Steroids
RVH/RV strain
Enlarged pulmonary arteries
Pulmonary HTN
Tx for pulm HTN
CCB, anticoag, treat underlying condition
RV failure, RVH
Tall peaked P waves
Cor pulmonale
Tx for cor pulmonale
O2, fluid restriction, diuretics
Exudate vs. Transudate
Exudate=leaky capillaries
Transudate=increased pressure
Light’s criteria
Light’s Criteria: exudate if any of the following are true
- Pleural fluid protein to serum protein ratio >0.5
- Pleural fluid LDH to serum LDH ratio >0.6
- Pleural fluid LDH >⅔ upper limit of nl for serum LDH
Causes of left shift of PO2/Hb dissociation curve
↑ pH, ↓ temp, CO poisoning
Causes of right shift of PO2/Hb dissociation curve
↓ pH, ↑ temp
