A

The accident near miss form is used to dig deep into what happened to prevent an injury in the future and eliminate the root cause of the incident. Identify the situations and circumstances surrounding an accident or near miss. The form can be found in the safety center and front office. • The supervisor or manager of the partner involved in the accident or injury should complete the investigation. Follow up should be done with the affected partner and anyone who works in the same area.

A

Near miss reporting allows us to see our opportunities for improvement. It gets partners engaged in safety and taking responsibility for their work environment. Gets more eyes on the location to look for and identify concerns. Allows for coaching and retraining of partners and help reduce our chances of an accident

Explain the safety pyramid.

A

It represents the ratio between at-risk behaviors, near-miss events, and injuries by severity. At the base of the pyramid are at-risk behaviors which result in near Misses without adverse consequences. Recordable injury’s/lost workdays. Fatality. Doing something unsafe usually doesn’t result in an accident but the more times we allow unsafe actions to occur the wider the base of the pyramid and in turn increases the probability of an injury.

What is a Safety Data Sheet? (SDS)
Document identifying the chemical composition of substances in the facility. SDS sheets contain what the chemical is composed of, First aid, PPE required, What to do if exposed, Storage info and spill response

When a new chemical is being brought into the facility, by any department, it needs to be reported to the PM, the PM reviews and signs off on the chemical approval form. This form is added to the SDS binder

In binders located in the saftey center. These must be maintained in 3 ring binders, made available to all partners (not locked up), A separate binder is needed for Customer SDS listing customer chemicals that could be on uniforms or towels.
