What does the Health and Safety at Work Act 1974 require?
Requires employers to provide and maintain systems of work that are safe as far as is reasonably practicable

How are SSOW’s implemented?

When should SSOWs be reviewed?
- Following a major change (like equipment)
- Following an accident/incident
What is the purpose of a risk assessment?
Ensures safety of personnel by identifying hazards and mitigating risk
What does the law say risk assessments should show was done by the assessor?
- A proper check was made
- You asked who might be affected
- You dealt with all obvious significant risks
- The precautions are reasonable
- You involved the workers in the process
What is the 5-step process for a safety risk assessment?
- Identify hazards
- Who can be harmed
- Evaluate risks, identify control measures
- Record findings
- Review/update as required
When is a risk assessment required?
- There is no SSOW
- The existing SSOW is inadequate
- There is a requirement to operate outside the SSOW
- The activity is particularly hazardous
- When required by a PTW
- When the supervising person deems it necessary
Who should conduct risk assessments?
Individuals responsible for supervising the work, assisted by the DSR
Where are risk assessments should recorded?
MOD Form 5010 (BRd 167)
Who signs a MOD Form 5010?
- Assessor
- HOD
Where is guidance on PTWs laid down?
- JSP 375
- BR 375
When is a PTW required?
There is still a significant level of risk after a SSOW is used
What activities require a PTW?
- Connecting/disconnecting shore services
- Diving
- Hot work
- Turning shafts
- Embarking/disembarking waste fluids
- Confined spaces
- Fin/casing work
When would a generic PTW be used?
When work is not covered by an existing PTW, but a PTW is still required
When would a generic PTW be used?
- Working at height
- Working on live electrics
- Working on inert gases
Where are generic PTWs recorded?
MOD Form 5071
What is the Ship Staff Work Sponsor responsible for?
- Putting control measures in place
- Overseeing the activity
- Acting as point of contact for contractors
Where is the PTW Log held?
- By the SCC
Where is Tagout information laid down?
BRd 23 TISMs (Technical Instructions for Submarines)
Who is responsible for tagouts?
- ATR (OPS LET)
- Non-OPS LET (completed a board with HOD + 2 training tagouts)
Where can you see all ATR personnel?
- Tagout log
- MATSTAT
What needs to be done if an ATR needs to transfer responsibility of a Tagout to another ATR?
- Formal handover must take place, including a full audit of all tags
- The oncoming ATR signs Tagout form to accept responsibility
Who authorised tagouts at sea?
- MEO
- HOD
