Where do workers sign on the Tagout form to say they are happy to work on a tagged out system?
WORKER DECLARATION
28
What is the difference between working aloft/working at height?

31
What are some main hazards when working on live electrical equipment?
- Electric shock - Arcing - Fire - Explosions - Static shocks - Electric shocks
32
What are some main causes of arc flash?
- Human error (improper training, mistakes, etc) - Equipment failure (lack of maintenance, faults, etc)
33
What type of PTW should be used when working on live equipment?
Generic PTW
34
What precautions should be taken when working on live electrical equipment?
- 2 man rule - PPE + rubber mat - Insulated tools - Electrostatic straps
35
Where is guidance on working in noisy environments laid down?
- BRd 167 - JSP 375
36
What needs to be done before working in a noisy environment?
Ship’s Noise Assessor should conduct a noise risk assessment
37
What are some consequences of noise hazards?
- Tinitus - Temporary hearing loss - Permanent hearing loss
38
What is meant by a confined space?
An enclosed or partially enclosed space where there is a foreseeable risk
39
Who is responsible for safe working in a confined space?
- Working party - Rescue team - Authorised Person Confined Spaces - Person in charge - Sanctioning Officer
40
What precautions need to be taken when working in a confined space?
- Ventilation - Communications - Gas monitoring - EDBA/EBS
41
What hazards exist in a confined space?
- Atmosphere spec - External hazards (systems, water movements, etc)
42
What are some key roles in hot working?
- Hotworker - Hotwork sentry - Owner of the task
43
Where is guidance on hotworking laid down?
- BRd 167 - BRd 9902 SMGOs
44
Where is hot work prohibited without specific approval/certificates?
- Confined spaces (need Safe to Burn cert) - Platforms with high voltage cabling (unless approved or isolated)
45
What should be considered before doing hot work?
- Equipment is suitable/calibrated - Flammables removed - Explosion risk reduced (BRd 862) - Atmosphere monitoring - Ventilation - Firefighting
46
47
What does the hot work sentry need to sign for before hot work can take place?
Hot work sentry brief to say they have been briefed and understand their role
48
What is the purpose of a Tagout form?
Provided an auditable document record of isolations put in place and permissions given prior to start of work
49
What is the purpose of the Tagout process?
Supports management and testing of equipment following repair/maintenance
50
What is the purpose of a Tagout label?
Provides an easily identifiable means of showing where isolations have been placed
51
Who is the “person conducting the Tagout”?
A technical rating conducting the Tagout, delegated by the ATR ATR is still responsible for ensuring the system is isolated correctly/returned to a normal line up
52
How do you isolate electrical supplies to equipment?
- Identify supplies using composite diagrams - Present Tagout form to HOD/MEO for approval - Remove power to equipment using relevant switches/breakers - Place Tagout label clearly
53
What is the Tagout process for pressurised mechanical systems?
- Identify 2 valve isolations to be made using composite diagrams - Present isolations to MEO/HOD - Ensure system is vented downstream - Attach Tagout labels
54
What must be done if no SSOW exists when restoring a tagged out system?
A method statement should be attached to the Tagout form
55
Give an example of when a tagged-out system restored would still be designated at “awaiting test”?
A repair to the Weapons Discharge System may be restored but the WEO may want the wider system tested before signing the Tagout form off
56
What happens to tagged out systems that are ready to be tested, but another Tagout boundary means it cannot be tested?
It is designated as “awaiting test”
57
What is the process for removing a Tagout?
ATR ensures all labels are accounted for and presents them to the MEO/delegated representative. The tags can then be destroyed, and the Tagout form retained for audit purposes
58
What should be annotated on the Tagout form after the tags have been destroyed?
“All tags sighted and destroyed by XXX on XXX. System tested correct and returned to normal lineup”
59
How long do completed Tagout forms have to be archived for?
Minimum 1 month then destroyed by HOD/DMEO
60
How long should Tagout log index pages be archived for?
Until the end of the Operating/Maintenance period, then destroyed at the beginning of the next period
61
When would a Tagout go in the long term Tagout log?
If a Tagout exceeds 1 operating/maintenance period
62
What is a safety Tagout?
A Tagout that makes a system safe due to a defect, using a single valve isolation to prevent it being reenergised (eg electrical failure, bridge fin safety wire)
63
Can work be conducted on a safety Tagout?
