A

code 6- Independent- resident completes the activity by themselves with no assistance

Code 5- Setup or clean up assistance- helper Sets up or cleans up; resident completes activity.

Code 4- helper provides verbal cuing or touching/steadying

Code 3- Helper does less than half of a task

Code 2- Helper does more than half of a task

Code 1- Helper does all of the task.

Code 7- resident refused to complete activity (really don’t want code 7 on form)

Code 9- not applicable (patients wasn’t doing the task before they came to the hospital)

Code 88- not able to effectively evaluate due to inability to breath and perform. Unstable
