How to diagnose
D-dimer test
What is 1st line VTE treatment
Apixaban or rivaroxaban
What is 3rd line for VTE treatment
LMWH + warfarin for 5 days until INR is >2 for 2 consecutive readings
Then warfarin alone
What is 2nd line VTE treatment
LMWH for 5 days then dabigatran or edoxaban
Duration of provoked DVT treatment
Recent risk factor in last 3 months
3 months
Duration of unprovoked DVT
No recent risk factors
More than 3 months
Duration of treatment with active cancer
3-6 months
Duration of treatment for recurrent VTE
Long term
What is VTE prophylaxis when to start
Mechanical - compression stockings
Anticoagulants - start within 14hrs of admission
After surgery what is VTE prophylaxis plan and durations
LMWH or fondaparinux for atleast 7 days post surgery or until mobile
28 days major cancer surgery in abdomen
30 days in spinal surgery
VTE prophylaxis for elective hip replacement plans
LMWH for 10 days then 75mg aspirin for 28 days
LMWH for 28 days then stockings until discharge
Rivaroxaban if not then apixaban or dabigatran
VTE prophylaxis for elective knee replacement plans
75mg aspirin for 14 days
LMWH for 14 days then stockings until discharge
Rivaroxaban if not then apixaban or dabigatran
VTE pregnancy after birth, miscarriage or termination in past 6 weeks
Start LMWH 4-8hrs after event and continue for minimum 7 days
Treatment of VTE in pregnancy
LMWH
UfH if high risk of haemorrhage
