D-Dimer - elevated in many problems hence sensitive but not specific

  • if negative - high true dont have VTE
  • if positive - low specificity for VTE specifically

- if have clot, veins less collapse

Steps for Investigations for PE

A

Check pre-test probability cos NPV of DDimer is inverse to prevalence;

  • DDimer negative may be flawed if prevalence is high
  • Wells criteria

if likely, prevalence is high, NPV is low - go for Scan

  • V/P(Q) Scan
  • CT Pulmonary Angiogram
