How many bones are there in the foot and ankle?
26 bones, or 28 if you include the sesamoid bones at base of big toe
7 tarsals
Divided into 3 units, forefoot/mid foot/hindfoot
What are the joints of the foot? (9)
- PIP
- DIP
- metatarsophalangeal joint
- tarsometatarsal joint
- intercuneiform joint
- naviculocuneiform joint
- talonavicular joint
- calcaneocuboid joint
- subtalar joint
What are the tarsal bones?
Medial, intermediate and lateral cuneiform Cuboid Navicular Talus Calcaneous
Where is the lisfranc joint complex?
Where the metatarsals articulate with the tarsals
Where is the chopart joint complex?
At the talonavicular joint and calcaneocuboid joint
What are the 3 different sections of the foot?
Forefoot, midfoot and hindfoot
What are the indications of the 2.7/3.5 VA forefoot midfoot set?
For fixation of osteotomies, fractures, malunions/nonunions of small bones and bone fragments in adults and adolescents. Especially in osteopenia bone
What size compression wires does the foot set have?
1.6 K wires (150mm in length)
What size do the compression wires come in?
1.6 in diameter
Threads go from 10-40mm in 5 mm increments
15mm is commonly used on foot
What size holes do the VA holes accept?
2.4 and 2.7 Va locking screws
If using 2.4 make sure 1.8 drill bit and guide is on set
Non VA can be used but only nominal angle
Proximal and distal reamers are used for what?
To prepare the articulating joint surfaces- in particular the first metatarsal
How are the distal reamers used?
They are cannulated so they can be used over a K-wire
6-12mm reamers accept 1.25 K wires
14-24mm reamers accept 1.6 k wires
What shape are the reamers?
The proximal reamer is concave
The distal is convex to form a pocket in the joint surface
What size do the reamers come in?
The 1.25mm k wire reamers come in 6,8,10 and 12 diameter
The 1.6mm k wire reamers come in 14, 16, 18, 20, 22, 24
What are first MTP fusion plates indicated for?
Arthrodesis of the metatarsalphalangeal joint
What size do the 1st MTP plates come in?
Small and medium plates have 3 different angles of dorsiflexion (0, 5 and 10 degrees.
Large plates have one size of 5 degrees dorsiflexion
Revision plates has 0 degrees of dorsiflexion and is identified by the star (va hole) smack bang in the middle of the plate
What are some features of the 1st MTP plates?
- all plates come in L and R with 10 degrees of valgus
- etched with a line to help with placement over fracture site
- k wire holes
What are First TMT fusion plates indicated for?
First tarsometatarsal arthrodesis
Where can the TMT plates be placed?
The TMT joint, second or third or for LISFRANC fractures
What sizes do the first TMT plates come in? What do they look like?
Standard and long (elongated combi hole distally)
They have a little head with equal angle arms and legs either side
What sizes do the TMT plates come in?
What does it look like?
ONE SIZE!
43mm in length
2 bug eyes in the head with no arms or legs
What screws do the TMT and MTP Plates take?
- 4mm VA locking and cortex
2. 7mm VA locking and cortex
What instruments are needed to set up a foot set?
- K wires (1.6) and reduction forceps, compression wires
- 1.8 drill and drill guide for 2.4 screws (purple)
- 2.0 drill and drill guide for our 2.7 screws (orange)
- measuring gauge (probs black-up to 40mm)
- screwdriver will be T8 for both and 1.2 torque limiter
