What do you do when you suspect varicose veins? Describe the test

A

Trendelenburg Test

  • elevate limb as far as possible and milk, then occlude the saphenofemoral junction.
  • pt swing over to side and stand
  • variscosities refill immed. –> perforating veins further down leg
  • no imm. refill –> incompetence @ saphenofemoral junction

What do you do when you suspect peripheral arterial disease? Describe the test

A
Buerguer's Test
- ask if any pain or restriciton in hips
- elevate feet to 90º
- watch pallor
normal --> pale @ 90º
diseased vessels go before (50 severe & 25 critical ischaemia)
- pt bring leg over side of bed 
- observe feet 
- PVD feet engorge + purple, then deep red (reactive hyperaemia)

A

Triphasic - normal
biphasic - atherosclerosis
monophasic - severe vascular disease
