1

Anti-GBM

capillaries

2

Henoch-S purpura

venules-->arteriole (TINY)
child
IgA
purpura, arthritis, GN, abdominal pain
(skin, joint, kidney, GI)-->PAGAN
purpura lower extremities

3

Cryoglbulinemmic

venules-->arteriole-->bigger
Immune complex
Hep C
IgG and IgM
RF positivity (type II and III)
purpura w/ ulcers
peripheral neuropathy and Raynaud's
Secondary sjogren's

4

Leukocytoclastic
(AKA Hypersensitivity angiitis...see other NC)

Venule-->capillary-->arterioles
spares internal organs
Meds or infections
similar to serum sickness

Hypocomplementemic urticarial vasulitis
Connective Tissue Disease Assoc (CTD)
Rheumatoid Vasculitis
Erythema Elevatum Diutinum-->skin, extensor hands and knees

5

Microscopic polyangiitis

Venule-->small-sized artery--> medium sized
Kidneys (GN), Lung, Neuropathy, Skin
PANCA

6

Granulomatosis w/ Polyangiitis

Vein-->small-sized artery
granulomas
White guy
Upper and lower respiratory tracts
kidneys (ENT, lungs, kidney)
saddle nose
CANCA (Wegener was a NaCi..Nazi) (PR3)

histo has rheumatoid nodule-like spots
geographic necrosis

7

Churg-Strauss syndrome
AGPA

Vein-->small-sized artery
granulomas
PANCA (Churg-Strauss was NOT a Nazi)
peripheral neuropathy
GU involvement (i.e. prostate)

Wegener's + eosinophils

8

Polyarteritis nodosa

medium sized arteries-->aorta
male
Hep B (changes Rx...antivirals & plasma exchange)
neuropathy
Kidney involvement (NOT GN/ RBC casts though)
GI, testicular, cardiac, skin

SPARES lungs

fibrinoid necrosis

cutaneous version (Hep C)

Can confuse w/ RV

Acute: PMNs +/- thrombus, occlusions, rupture
branchpoint of arteries
Chronic: fibroblasts +/- aneurysms

LESIONS AT DIFFERENT PHASES!!
ST

9

Kawasaki dz

medium sized arteries-->aorta
asian child
coronary arteries
immune activation and cytokines
fever, rash, bilateral eyes, LAD, extremeties, mucus membranes (i.e. strawberry tongue!)--> cardiac problems most mortality
lymphocytosis
Aspirin

endothelial necrosis
aneurisms

10

Buerger's dz
thromboangiitis obliterans

arteriole-->medium/muscular artery
granuloma
young male smokers
lower limbs
Antibodies against ECM and endothelial cells
ANCAs

ACUTE: no Necrosis
Chronic: organization of thrombus

11

Giant cell arteritis

aorta and primary branches
granuloma
>50 white female
polymyalgia rheumatica assoc/overlap\
empirical prednisone Rx
IFN-y
headaches+ visual+ jaw

lymphocytes
ST

12

Takayasu's

aorta and primary branches
granuloma
thin)
ST

13

Cogan's Syndrome

aorta and primary branches
granuloma
young adults
painful eyes/ hearing loss
aneurysm

DON'T confuse w/ Susac's syndrome

14

Susac's syndrome

endotheliopathy
sudden onset of hearing loss and eye probblems

15

Hypersensitivity vasculitis

Small vessels
immune complex
SLE, Sjogren's, RA assoc
Cutaneous involvement!!-->palpable purpura esp.

Med or infection rxn

16

Primary Angiitis of CNS

brain, meninges, spinal cord
Rule out mimics (i.e. RCVS)

Granulomatous angiitis of CNS=subset in middle-aged men
Atypical CNS= subset w/ lymphocytic rather than granuloma

17

Behcet's dz

complex, multi-system
oral and genital ulcers
eyes and skin
Silk route epidemiology
young
HLA-B51
neutrophilic vascular rxn
