What are the risk factors of vulvar cancer?
- Cigarette smoking
- HPV infection (60%)
- Immuno-suppression
- CIN or VIN
Symptoms of vulvar cancer
Pruritus, bleeding, discharge
What will you see on exam?
Unifocal vulvar lesion- plaque, ulcer, mass
How is vulvar cancer diagnosed?
VIA- Biopsy
Name the 3 types of vulvar cancer.
Squamous, Melanoma, Basal cell
What are the 3 types of squamous vulvar cancer?
Differentiated type, Classic type, Verrucous
In which kind of women are melanomas and basal cell cancers common in?
Post-menopausal and white women
How do you manage stage I or II?
Preferred: Definitive radical local excision + inguinal lymphadenectomy
OR Radiation therapy
For those who are NOT surgical candidates
Adjuvant therapy for high-risk tumors:
Size > 4cm, lymphovascular invasion, close or positive surgical margins, + lymph nodes
How is vulvar cancer staged?

How does vulvar cancer spread?
Direct extension
Lymphatic- Inguinal, Ipsilateral spread
Hematogenous
How do you manage stage III or IVA?
Surgical resection of primary tumor and lymphadenectomy- May require resection of bladder, urethra, rectum
Primary chemoradiation for Non-surgical candidates
How do manage stage IVB?
Primary chemotherapy
Palliative care
