Man aged 60 presents with haemetemysis, haemetegelsia, a raised urea and dysphagia. He has a diet low in fibre, smokes and upon investigation has an infection with H. Pylori. Diagnosis?
Gastric ulcer
Gastric cancer
Man is diagnosed with squamous cell carcinoma. Treatment?
Sensitive to radiotherapy and chemotherapy
Man is diagnosed with adenocarcinoma what is the treatment?
Chemotherapy then surgical resection
You suspect gastric cancer, investigation?
Endoscope PET scan (mainly to see if metastasised)
What are the four types of diarrhoea?
Secretory
Hypermobility
Osmotic
Exudative/inflammatory
What can cause secretory diarrhoea?
Cholera by stimulating CTFR that secretes Cl- ions into lumen causing sodium and water to follow
What condition causes hypermobility diarrhoea?
Coeliac disease
Reduced transit time causing malabsorption and diarrhoea
What condition can cause osmotic diarrhoea?
Glucose/ galactose malabsorption syndrome
Due to defective ion transport causing solutes in lumen to fail to be broken down meaning it is hypertonic and water will flow into it.
What can cause exudative (inflammatory) diarrhoea?
Crohns, ulcerative colitis, E.coli, infective colitis
What can cause pseudomembraneous colitis?
C. Difficile infection
Can be secondary to antibiotic use
Define acute liver disease
Rapid development of hepatic dysfunction without prior liver disease lasting less than 6 months
What two vessels supply the liver?
Hepatic artery
Hepatic portal vein
Women presents with an itch secondary to primary biliary cirrhosis, what do you prescribe?
Uresodeoxycholic acid
Young women presents with rapid onset of jaundice and encephalopathy. You discover she has overdosed on paracetamol. Treatment?
N- Acetylcysteine which is then broken down into glutathione that neutralises the toxic metabolites of paracetamol
What is fulminant hepatic failure?
Rapid development of acute liver injury with severe impairemnt of synthetic function and hepatic encephalopathy in patients without previous liver disease
