Wound Assessment and Management Flashcards
(22 cards)
Definition of a wound
break or opening in the surface of the skin
what are the 3 types of wounds
-surgical
-traumatic
-chronic
define a surgical wound
intentional cut to skin
define a traumatic wound
sudden, unplanned injury
define a chronic wound
been present for 6 weeks or more. can be result of a non-healing surgical or traumatic wound, or a pressure ulcer
what is an acute wound
short duration, heals without complication
what are the 4 stages of wound healing
- haemostasis
- inflammation
- granulation
- epithelialisation
what is the rhyme for wound healing
henry isn’t good at eating
what is haemostasis
blood vessels contract, platelets aggregrate and a clot is formed. leucocytes are attracted to injury area
what is inflammation stage
proteins are released, causes vasodilation and inflammation. neutrophils and macrophages multiply in the wound
what is the granulation stage
new supporting tissue is formed. new blood vessels develop and wound begins to contract
what is the epithelialisation stage
new skin cells emerge from dermal edge slowly bringing wound together
what are the 3 types of wound healing
primary , secondary and tertiary intention
what is primary intention healing
straight wound closure, edges of wound can be brought together. i.e. stapling, suturing
what is secondary intention healing
edges of wound cant be brought together, heals by wound contraction. i.e. pressure ulcer
what is teritary intention healing
combination of both
intrinsic factors affecting healing
cant be fixed immediately- age, chronic disease, dehydration
extrinsic factors affecting healing
can be avoided - pressure, smoking, poor wound care
what is necrotic tissue
black dead tissue
what is sloughy tissue
white, yellow, leaking tissue
what is granulated tissue
pink, healthy
what is epitheliazing tissue
skin starting to grow over
