Drugs for Hemophilia Flashcards
hemophilia
a genetic bleeding disorder is seen almost exclusively in males; platelets aggregate correctly, but fibrin formation does not
Hemophilia A:
deficiency of clotting factor VIII (aka antihemophilic factor)
6x more prevalent
Hemophilia B:
There is a deficiency of clotting factor IX (aka Christmas factor)
Cornerstone of treatment is replacement therapy:
Factor VIII (for hemophilia A) Factor IX (for hemophilia B)
based on prevention
Desmopressin (for mild)
Promotes the release of factor VIII
Much cheaper than Factor VIII
med
Disadvantage: can deplete stored factor VIII
Antifibrinolytic drugs:
Aminocaproic acid
Pain meds:
Acetaminophen or opiates
Avoid NSAIDs and Aspirin
Factor VIII-hemophilia A
Recombinant Factor VIII is safer than plasma-derived factor VIII (donor factor), but more expensive.
actual weight is needed to calculate dosage
Factor VIII-hemophilia A
side effects
Hives, rash, stuffy nose, fever GIVE ANTIHISTAMINE (diphenhydramine)
Factor IX-hemophilia B
recombinant and donor forms available; Recombinant is safer; IV push at first signs of needs accurate weight
Hemophilia Therapy-Desmopressin
Antidiuretic hormone
stop/prevent bleeding w/ mild depression; VIII for HA
Hemophilia Therapy-Desmopressin side effects
Fluid retention
Hyponatremia
