Acute care hospital
Inpatient
- LOS: 1-7 days
- eval: quick/accurate (cognitive status, safety at home)
- tx; stabilization, client/caregiver ed, motivation to peruse follow up after discharge, d/c plan
Subacute/intermediate care
Stabilized from acute care, but not yet stable for outpatient care (can be in hospital or SNF)
- LOS: 5-30 days
- eval: more in-depth assessments, observation
- tx: d/c to expected environment
Long term acute care hospital
Chronic, catastrophically ill, extensive care, life support/ventilation
- multiple dx
- LOS: 25 days +
- eval/tx: palliative care, prevent complications (if cognitively intact: mastery of environment, goal attainment)
Rehabilitation hospital
Medically stable with residual deficits (d/c to SNF, supportive community residence, home or independent living)
- LOS: week-months (ends when insurance coverage ends)
- eval: can be extensive
- tx: functional improvement, compensatory, modify d/c environment
Long term hospital
Chronic, can’t be treated in outpatient
- LOS: month-years
SNF/extended care
Stable, chronic, needs skilled care
- LOS: month-lifetime
Forensic settings
Criminals (jail, prison, forensic psych unit/hospital)
- LOS: via court order
Outpatient
No hospitalization, stable
Residential programs
Deficits which affect independent living
- 24/hr supervised quarter way houses
- halfway houses
- group homes
- supportive apartments (these three all have weekly/biweekly check ins)
Eval: living skills
Tx: ADLs, environmental mods
Partial hospitalization
Day hospital
- living home or halfway house but need tx
- LOS: 1wk-6m
Adult day care
Group tx, adults/elders with chronic issues or frail
- LOS: indefinite
Hospice
Client has 6 months or less to live
