Fall Prevention Interventions
A
- Make sure other personnel are aware of fall risk
- Keep bed in low position with bed rails up
- Bed wheels locked
- Bed and Chair alarms on
- non-skid footwear
- Floor free from clutter and clear path to bathroom
- Adequate lighting (night-light)
- Make sure client knows how to use call light and grab bars
- Orient client to the setting
- Keep assistive devices near by
- Educate client and family on risks and plan of care
- Keep client close to nurse’s station
- Answer call lights immediately
- Report and document all incidents
- Diuretic patients –> keep bedside commode to limit the amount of steps taken due to fall risk

Seizure Precautions
A
- Client with a history of seizures should be assessed for frequency (how long they last), type (partial or generalized) and date of last seizure, medications, and triggers
- Before seizure:
- keep bed in lowest position
- pad bed rails
- rescue equipment at bedside (oxygen, oral airway, and suction)
- During seizure
- place client on side with head slightly flexed
- do not place anything in mouth, except for suction if needed
- make sure airway is patent
- loosen closing
- Time and observe the seizure
- measure pulse and respiratory rate
- After seizure:
- document in medical record (length, LOC, respiratory status, color, limb movement, response)

Radiation Precautions
A
- keep the most distance possible from source of radiations
- minimize the amount of time near the radiation source
- wear LEAD gloves and aprons
- wear radiation (dosimetry) badges to monitor the amount of radiation exposure
- do not touch radioisotope containers
- Client with an implanted radioisotpe should be in private room with a private bath
- dispose of bodily fluid in marked container
- Use long-handled forceps to move any dislodged implants

Fire Safety
A
- RACE acronym:
R: Rescue patients (evacuate patients)
A: Alarm the fire (set the fire alarm)
C: Contain the fire (close the door)
E: Extinguish the fire - Classes are used to determine the type of fire extinguisher to put out the fire:
Class A: paper, wood, rags, trash
Class B: flammable liquids and gases
Class C: electrical fires
-How to use a fire extinguisher (PASS acronym):
1. P: Pull the pin
2. A: Aim at the base of the fire
3. S: Squeeze the handle
4. S: Sweep the extinguisher side to side

Triaging Clients
A
- Emergent Category I: highest priority given to those clients who have life-threatening injuries but also have high chance of survival
- Urgent Category II: Second highest priority given to clients who have major but not life threatening injuries and can wait 45 - 60 minutes
- Non-urgent Category III: Clients who have minor injuries and do not need immediate attention
- Expectant Category IV: clients who are not expected to live and will be allowed to die naturally
