Adenoma
What is a papilloma
This is a benign neoplasm in squamous epithelium
It is to a squamous carcinoma what a adenoma is to a adenocarcinoma
Adenocarcinoma
You are presented with a case decribing the features of a neoplasm in the colon
- Invasion into the stalk or colonic wall
Malignant: Invasion!!
Metastasis to liver would indicate that it is malignant however that wasn’t presented to you and so isn’t really answering the question at hand
You are presented with a case where a man had a history of colonic cancer.
A sample is taken from a nearby lymph node where an adenocarcinoma is found
- Lymphatic metastasis
Haematogenous - throug the blood vessels - distant site
Transcloelomic - bursts through the coelom and into the abdomen. Coelom is the cavity containing the digestive tract
Could also be directly from colon to an adjacent organ e.g. bladder
A woman who has a history of treated adenocarcinoma in the colon now has had weight loss.
Examination with a CT shows serveral nodules on the liver.
This are removed surgically and sent for pathology.
They show prescence of large areas of adenocarcinoma within the liver itself
A: Haematogenous
Likely through the portal venous system
Could be transcloelomic or direct spread however this would be present on the surface of the liver not inside.
Additionally in transcloemic you would see the part of the colon affected stuck to the liver during the surgury.
When a cancer metastasises what happens to it’s classification?
As far as I can tell it stays the same
If it started out as an adenocarcinoma in the colon then spread to a lymph node it is still an adenocarcinoma in the lymph node not a lymphoma
