Regional Anesthesia

A

Blocks!
Nerve blocks for one specific area
-spinal block
-can be used with moederate or MAC
-through catheter
-stays in for only about 72 hrs
-disadvantage: insertion site and inadvertently puts it into the vascular system instead of the nerve.
-ANS and motor block…vasodilator.
-epidural block
-patient can remain conscious or they can also get anesthesia
-Danger in how high you put it…risk for meningitis post epidural.

Malignant Hyperthermia

A

Rare and scary.
Complete skeletal muscle rigidity and hyperthermia
-Only certain anesthetics…
-triggered by stress and trauma and heat.
-Genetic link: ask hx
-Hypermetabolic state due to Intracellular calcium inside.
-1st sign- pt. tenses up during surgery or has muscle contractions.
-Then…tachycardia/tachypnic. High Co2, Ventricular dysrhythmias, resp. acidosis, metabolic acidosis,
-LATE sign: very high temp.
-Use anesthesia
