• Do you have any trouble moving around at home? do you have stairs and can you mange them?
• Feeding - able to manage at meal times?
• continence - do you manage going to the bathroom okay on your own or do you need assistance?
• have you any difficulty with having a shower?
• have you any difficulty getting dressed in the mornings? any difficulty with buttons/ shoes or socks?

• Do you need any help managing your prescriptions or all the different medications you use?
• Do you cook at home or get help to prepare meals?
• can you do bits of housework and clothes washing and cleaning at home or do you get help?
• would you walk to the shops or get the bus?
