Mod in combo with beta-lactam or glycopeptide: GP cocci

3

Gentamicin/Tobramycin Dose

IV/IM, MDD: 1-2.5 mg/kg/dose q8-12h, Extended: 5-7 mg/kg

4

Amikacin Dose

IV/IM, MDD: 5-7.5 mg/kg q8-12h, Extended: 15-20 mg/kg

5

AG Kinetics

Conc dependent bactericidal activity

6

EID Renal

7

EID Contraindications

Severe burn victim, CF, pregnancy, neonates, peds, ascitesm HD

8

AG Dosing Weight

Base on IBW or AdjBM

9

AG Ke

Ke = 0.00293 x CrCl + 0.014

10

AG Vd

Vd = 0.23 L/kg x IBW

11

Gent/Tobra MDD Peak

Draw 30 min after infusion ends, Cmax = 5-10 mcg/mL

12

Gent/Tobra MDD Trough

Draw 30 min before dose, Cmax = 1 mcg/mL

13

MDD Interval

Interval = ((ln [Cmax/Cmin])/Ke) + t(inf)

14

MDD Dose

MD = [Cmax x (Vd x Ke) x (1-e -Ke x Tint)]/ (1-e -Ke x tinf)

15

MDD Loading Dose

LD = Cmax (Vd x Ke)/(1-e -Ket)

16

Amikacin MDD Peak

Draw 30 min after infusion ends, Cmax = 15-30 mcg/mL

17

Amikacin MDD Trough

Draw 30 min before dose, Cmax = 3-4 mcg/mL

18

MDD Steady State

3-4 half lives

19

Gent/Tobra In AFR

Redose at 1 mg/mg once level below 2.5 mcg/mL

20

AG and HD

HD removes about 50% of AG conc

21

AG EID Levels

6-14 hr dose checks against Nomogram
