Good: GNs(Pseudo, Acinetobacter)
Mod in combo with beta-lactam or glycopeptide: GP cocci
Gentamicin/Tobramycin Dose
IV/IM, MDD: 1-2.5 mg/kg/dose q8-12h, Extended: 5-7 mg/kg
Amikacin Dose
IV/IM, MDD: 5-7.5 mg/kg q8-12h, Extended: 15-20 mg/kg
AG Kinetics
Conc dependent bactericidal activity
EID Renal
60 over: q24h, 40-60: q36h, 20-40: q48h
EID Contraindications
Severe burn victim, CF, pregnancy, neonates, peds, ascitesm HD

Base on IBW or AdjBM

Ke = 0.00293 x CrCl + 0.014
AG Vd
Vd = 0.23 L/kg x IBW
Gent/Tobra MDD Peak
Draw 30 min after infusion ends, Cmax = 5-10 mcg/mL
Gent/Tobra MDD Trough
Draw 30 min before dose, Cmax = 1 mcg/mL

MDD Dose
