What does Analgesic Nephropathy present with?

What is Papillary Necrosis?
Sloughing off of the renal papillae
What causes Papillary Necrosis?
toxins such as NSAIDs or sudden vascular insufficiency leading to death of the papillary cells which drop off the internal structure of the kidney
What must be present for Papillary Necrosis to occur?
Some degree of underlying renal damage
Clinical clues in patient hx for Papillary Necrosis?
Extra NSAID use with a hx of Sickle cell disease, diabetes, urinary obstruction or chronic pyelonephritis
How does Papillary Necrosis present?
Sudden onset flank pain, fever and hematuria
What is the best initial test for Papillary Necrosis?
Urinalysis showing RBC, WBC and possible necrotic kidney tissue
What is the most accurate test for Papillary Necrosis?
CT scan showing abnormal internal structures from the loss of papillae
How do you treat Papillary Necrosis?
No specific treatment
