Why is it crucial to identify comorbid complains in addiction? (3)
- Prevalent (estimated >50% have a comorbid disorder)
- Patients with comorbidity often have a worse prognosis
- Psych complains can be caused or exarcebated by substance abuse
Name the four interventions that include family members/friends in the treatment of addiction?
- Al-Anon
- Johnson intervention (“confrontational intervention”)
- CRAFT (Community Reinforcement and Family Training)
- BCT (Behavioral Couple Therapy)
What is the Johnson intervention?
Addicts are unexpectedly confronted by close relatives/friends
- Very confronting, seen as a “last chance” to reach out/rescue
- Relatively little empirical support
What is the AI-Anon intervention?
Based on the twelve steps of AA
- Help close relatives accept that they are powerless and help them “detach with love”
- Basically, avoid influencing the addict’s behaviour
What is CRAFT (intervention)?
Treatment in which close relatives are trained (ish) to adjust their behaviour in order to motivate the patient to start treatment (and improve their QOL)
- Based on CBT & MI
- Convincing scientific evidence
What are the two central principles of CRAFT?
- Positive reinforcement of desired behaviour by close relatives
- Not rewarding unwanted behaviour (that is, letting the person experience the negative consequences themselves)
What is BCT (intervention)? (3)
- Teach the couple effective ways to cope with substance (-related situations)
- Teach the partner to support/reinforce behaviour change/sobriety
- Improve relationship satisfaction and reduce stress
Why may relationship satisfaction an important factor in addiction (treatment)?
Appears to be a protective factor for relapse
Why is BCT not used much in practice?
It is rather intensive and expensive, thus only when individual therapy is not succesful (or there are severe relationship problems), is it recommended
