What are the classification of ACS normally split into (3)?
STEMI
NSTEMI
unstable angina
What differentiates NSTEMI from unstable angina?
Troponin level ONLY
What features may be seen in an ECG on someone who has an NSTEMI or Unstable angina?
ST depression
T wave inversion
What leads are involved in an anterior or anteroseptal infarction? Which vessel is occluded?
V1-V4, left anterior descending
What leads are involved in an inferior infarct? Which vessel is occluded?
II, III, aVF and right coronary artery
What leads are involved in an lateral infarct? Which vessel is occluded?
V5 and V6 and often aVL and circumflex artery
What leads are involved in an posterior infarct? Which vessel is occluded?
Reciprocal ST depression in the anterior leads (V1-V4) Right coronary artery
What conditions can have ST depression or elevation?
Subarachnoid haemorrhage or traumatic brain injury
What ECG changes would you see in a large PE?
T wave inversion in V1-4
What ECG changes would you see in Brugada syndrome?
ST elevation in V1-2
What is the cut off for PCI in a STEMI?
12 hours or cannot be done within 2 hours of call
What drugs must be loaded prior to PCI?
ONE OF:
Clopidogrel 600mg
Prasugrel 60mg
Tricagrelor 180mg
What drugs are used in fibrinolysis in a STEMI?
LMWH, unfractioned heparin, fondaparinux
What are indications for Fibrinolysis in AMI?
Presents within 12 hours but cannot get PCI in 2 hours is a STEMI
What are the contraindications to fibrinolysis?
Previous haemorrhagic stroke, ischaemic stroke within 6 months, CNS dysfunction or cancer, recent trauma/ surgery, active bleeding within last month, known bleeding disorder, suspected dissection
What drugs are given in the week following NSTEMI?
Fondaparinux 2.5mg and aspirin 75mg and an additional antiplatelet (clopidogrel), for secondary prevention: beta blocker, ACEi, statin
What DAPT should be considered in ALL MI cases?
Aspirin 75mg life long and Clopidogrel 75mg for one year
When would your patient qualify for ICD implant post MI?
Ventricular arrhythmia 24-48 hours post MI
What drug is given for bradycardia?
atropine
What is Brugada syndrome and who does it affect?
Autosomal dominant ion channel disorder, young SE Asian males
