Define coronary vessels
Supply heart with blood
Define coronary arteries
Stems from the aorta to supply heart muscle with oxygenated blood
Define coronary veins
Drains deoxygenated blood into the right atrium
Cardiac disease and maternity
-leading cause of maternal death in the UK
-women are mainly dying from acquired heart disease
-change in maternity population : older age, more obese, health conditions
Pre-pregnancy counselling for known cardiac conditions:
-MDT input
-consideration of if and when to get pregnant
-assess risk of cardiac conditions
Conditions where pregnancy is not recommended:
-pulmonary hypertension
-eisenmengers syndrome
-ehlers danlos vascular
-stenotic valves
-left ventricular failure
-severe aortic dilatation
Booking/antenatal considerations:
-MDT plan
-record all medications and use a risk vs benefit
-refer with any worrying symptoms even if they may be usual pregnancy symptoms
Intrapartum considerations
-can have SVD with limited 2nd stage
-HDU care
-good pain relief and fluid balance
-active 3rd
When to be concerned about chest pains:
-sudden onset
-SOB
-pain radiating down back,arm,shoulder and jaw
-opioid requiring
-abnormal obs
-tearing feeling
Investigating chest pain:
-bloods
-ECG
-Xray
Myocardial infarction
-heart attack
-blood flow via arteries is impaired
-usually due to fatty deposits
- 1:100,000
-risk factors: age, smoker, obesity, family history
Aortic dissection
-splitting of the aorta
-4X more common in pregnant population
-+++pain, tearing sensation
-no picked up with X-ray, ECG or blood tests
-risk factors: HPT, CHD, marfans disorder
-immediate surgery
Dilated cardiomyopathy
Muscle walls of heart become stretched so can not pump blood properly
Hypertrophic cardiomyopathy
Enlarged heart cells so the thicker the heart chambers so can’t not hold has much blood causing obstruction
When to be concerned about breathlessness
-abnormal obs
-sudden onset
-with tachycardia, chest pain
