What can undertreated hypothyroidism in pregnancy lead to?
Miscarriage, anaemia, small for gestational age and pre-eclampsia
What is the management for hypothyroidism during pregnancy?
Levothyroxine which can cross placenta and provide thyroid hormone to developing fetus. Needs to be increased during pregnancy by at least 25-50mcg. Treatment is titrated based on TSH level.
Why is serum TSH measured in each trimester and 6-8 weeks post partum?
To assess for long term hypothyroidism
What medications in hypertension need to be stopped?
ACE Inhibitors, ARBs, Thiazide & thiazide like diuretics
What hypertensive medications are not known to be harmful?
Labetalol, CCBs, Alpha blockers
Why do women with epilepsy need to take folic acid 5mg daily?
Reduce risk of neural tube defects
Why do seizures get worse in pregnancy?
Additional stress, lack of sleep, hormonal changes and altered med regime. But not harmful to pregnancy.
What anti-epileptic drugs are safe to use?
Levetiracetam, lamotrigine & carbamazepine
What anti-epileptic drugs are avoided?
Sodium valproate - causes neural tube defects & development delay. Phenytoin - causes cleft lip & palate.
What medications are contraindicated in rheumatology patients?
Methotrexate
What medications are considered safe in RA patients?
Hydroxychloroquinine, Sulfasalazine, Corticosteroids for flare ups
