What is postnatal depression?
Low mood in the postnatal period
What types of low mood are there?
Baby blues → seen in majority of women in the first week or so after birth
Postnatal depression → seen in 1/10 women, starts within a month, with a peak around 3 months after birth
Puerperal psychosis → seen in about 1/1000 women, starting a few weeks after birth
What are symptoms of baby blues?
Mood swings
Low mood
Anxiety
Irritability
Tearfulness
What causes baby blues?
Significant hormonal changes
Recovery from birth
Fatigue & sleep deprivation
Responsibility of caring for the neonate
Establishing feeding
All other changes & events around this time
What is the treatment for baby blues?
Usually resolves within 2 weeks of delivery - don’t need treatment
How does postnatal depression present?
Low mood
Anhedonia
Low energy
How is postnatal depression treated?
Mild cases → additional support, self help & follow up with their GP
Moderate cases → SSRIs & CBT
Severe cases → input from specialist psychiatry services
What is the screening tool for postnatal depression?
the Edinburgh postnatal depression scale
10 or more = postnatal depression
What is the treatment for puerperal psychosis?
Admission to mother and baby unit
CBT
medications
Electroconvulsive therapy
How can you prepare for postnatal depression during pregnancy?
Women who have existing mental health conditions are referred to perinatal mental health services for support so a plan is put in place for after delivery
how does puerperal psychosis present?
- mood swings (similar to bipolar disorder)
- disordered perception eg. auditory hallucinations
