What is uterine rupture?
A complication of labour where the myometrium ruptures
What happens in an incomplete rupture?
The uterine serosa (perimetrium) surrounding the uterus remains intact
What happens in a complete rupture?
Serosa ruptures along with the myometrium and the contents of the uterus are released into the peritoneal cavity
What does it lead to?
Significant bleeding and baby may be released into the peritoneal cavity so high morbidity for baby & mother
What is the main risk factor for uterine rupture?
Previous C section- scar on uterus becomes a point of weakness and may rupture with excessive pressure
What other risk factors are there?
VBAC, Previous uterine surgery, Increased BMI, High parity, Use of oxytocin, uterine hyperstimulation due to vaginal prostaglandins
How does it present?
Acutely unwell mother and abnormal CTG with signs of: Abdominal pain & vaginal bleeding, Ceasing of uterine contractions, Hypotension & tachycardia, Collapse
What is the management?
Resuscitation & transfusion may be required
Emergency C section
