looks like Michkey mouse –> the ears are the red nuclei and the mouth is the cerebral aqueduct
How do you locate a lesion?
look for CN sings –> lesion location/side will be at the highest CN affected

Pt. presents with external strabismus or the right eye, loss pf pain and temperature from his left face/body, loss of proprioception from his left body, loss of pain and resting tremor of his left limb. What is the dx and where is the lesion?
- Benedikt Syndrome
- Right Midbrain
- hallmark is CN 3 palsy
How do you differentiate between CPA Syndrome and Lateral Medullary Syndrome?
CPA will have deafness, and LMS will have dysphagia
CPA – Pontomedullary junction – CN 6, 7, 8
LMS – Lateral Medulla – CN 9, 10
Pt. presents with internal strabismus of the right eye, inability to completely close their right eye, inability to wrinkle forehead on the right, and paralysis of lower face muscles on the right. What is the dx and where is the lesion?
- Millard Gubler Syndrome
- Right Pontomedullary Junction
- CN 6 + 7 involvement
