  1. age: men ≥45yrs; women ≥ 55yrs
  2. family history
  3. physical inactivity: not performing at least 30min of moderate PA at least 3 days/week for at least 3 months
  4. smoking
  5. obesity: BMI: ≥30kg/m2 or WC 102cm for males; 88cm for females
  6. hypertension: ≥140/90 or on antihypertensive medication
  7. dyslipidaemia: LDL: ≥3.37mmol/L or HDL <1.04mmol/L or TC ≥ 5.18mmol/L
  8. diabetes: fasting plasma glucose ≥ 7.0mmol/L or 2h plasma glucose tolerance test ≥ 11.1mmol/L
