increasing amount of acetylcholine
What are the 3 medications to treat mild-moderate dementia?
Acetylcholinesterase inhibitors:
donepezil
Galantamine
Rivastigmine
What are some counselling points for donepezil?
Take at bedtime due to dizziness
Can cause diarrhoea and vomiting
What are some side effects of galantamin?
severe skin reactions (SJS)
palpitations
seizures
What are the side effects and advice for rivastigmine?
N&V
incidences of fatal overdoses = remove previous patches
How long should haloperidol and risperidone be used for?
no longer than 6 weeks
What are the C/I of haloperidol?
QT interval
Hypokalemia
Parkinsons Disease
Dementia with lewy bodies
What is the treatment of moderate-severe dementia
Memantine
What is used to treat aggravation in dementia?
antipsychotics - haloperidol and risperidone
What is 1st line (2) for dementia with lewy bodies?
Donepezil or rivastigmine
What is 2nd and 3rd line for dementia with lewy bodies?
Galantamine then memantine
What is used to treat vascular dementia?
ONLY acetylcholinesterase inhibitors
OR memantine IF comorbid with alzheimers, parkinsons, dementia with lewybodies
What shouldn’t be used for frontotemporal dementia?
NO Acetylcholinesterase inhibitors or memantine
What is 1st and 2nd line for treatment of parkinsons disease dementia?
Acetylcholinesterase inhibitors only
Rivastigmine licensed then memantine 2nd choice
What are the side effects of acetylcholinesterase inhibitors?
parasympathetic side effects - rest and digest
diarrhoea, urinary incontinence, bronchospasms, muscle weakness, vomiting, lacrimation, salvation, bradycardia
