ireg shaped lesion with pigmented border and diameter >6mm

purple/silvery plaques on extensors/scalp, dry skin, oncholysis + nail pitting
plaque psoriasis
widespread water drop papule on trunk/arm/legs, erythematous
guttate psoriasis
hyperkeratotic scaly lesion with raised/rolled edges, ulcerated, non-healing

intensely pruritic pale erythematous blanching plaques of superficial dermis
urticaria (welts/hives)
generalised vesicular rash in kids, fever + malaise
