risk factors
surgery trauma immobilisation cancer high BMI smoking varicose veins COCP/HRT preg cardiac failure severe infection severe dehydration previous DVT/PE nephrotic syndrome thrombophilias
ix if suspected DVT and likely using the 2 level wells’ score (a type of wells)
proximal vein USS within 4 hr-> D dimer if USS negative
-D-dimer test + interim 24-hour dose of a parenteral anticoagulant (if a proximal leg vein USS cannot be carried out within 4 hours) and a proximal leg vein ultrasound scan carried out within 24 hours
if DVT suspected and unlikely using 2 level DVT wells score
d dimer -> if positive do proximal leg USS or give parenteral anticoag and do USS within 24 h if can’t do within 4 h
what else raises d dimer
malignancy
pregnancy
surgery
what type of USS
duplex (combines traditional USS and doppler)
what to search for in pt with DVT
cancer!!!!
rx
LMWH or fondaparinux 5 days
give warfarin/DOAG long term (3 months at least, then assess risk)
give 3 doacs
rivaroxaban
dabigatran
apixaban
prophylaxis whist in hospital
early mobilisation
TED stockings
raise foot of bed
LMWH heparin
