what is cardiogenic shock
failure of heart to pump -> reduced CO
defined as:
sustained -hypotension <90 systolic for >30 mins
-tissue hypoperfusion (cold peripheries, oliguria)
what is cardiogenic shock usually a complication of
MI

circulatory collapse resulting in sever tissue hypoperfusion

treatment of shock
AB
C - venous access, send bloods, IV fluid 250ml boluses
monitoring
what is monitored in shock
cardiac monitoring, BP monitoring (usually via arterial line)
venous pressure monitoring (via central venous pressure)
urinary catheter
recognising shock
pulse weak and rapid
pulse pressure reduced
urine output reduced
low BP (but young people maintain BP v v well)
hypovolaemic causes of shock
loss of blood
dehydration
burns
pancreatitis
causes of cardiogenic shock
cardiac tamponade tension pneumothorax (squishes heart) PE (RV failure) MI fluid overload of preexisting poor particularly RIGHT ventricular function mycarditis
