what is endocarditis
infection of the endocardium, inner lining of the heart
high morbidity and mortality rates
what antibiotics are associated with the treatment of endocarditis
amoxixillin, gentamicin, vancomycin, flucloxacillin, rifampicin, benzylpenicillin sodium
what determines the abx used in the treatment of endocarditis
initailly blind therapy with amoxicillin (consider adding low dose gentamicin) OR vancomycin (if penicillin allergic)
when microbiology results come backwe can change treatment to a more suitable abx or abxs
MOA gentamicin
binds to 30S subunit of bacterial ribosomes = inhibition of protein synthesis
irreversible binding
often used in conjuction with beta lactams
MOA amoxixillin, flucloxicillin
Inhibits bacterial cell wall synthesis (binds to PBPs)
MOA vancomycin
Inhibits bacterial cell wall synthesis by binding to D-Ala-D-Ala terminus of peptidoglycan precursors
