Q

What transition is made if ESAS actuated on high RB pressure alone and a stem or feed leak is indicated?

Q

What action is taken if PZR spray line cannot be isolated?

A

If both valves fail to close and SCM adequate then trip all RCPs and do not restart them.

Q

What transitions are made if ESAS is not corrected (Step 12)?

A

1) If uncontrolled RCS cooldown due to HPI/break flow regardless of SG status go to small break LOCA c/d
2) If PZR level is rising without corresponding rise in RCS temp or press then go to forced flow or natural circ cooldown based on RCP status
3) If RCS leak is un-isolable and RCS pressure > 150# then go to forced flow c/d or natural circ c/d based on RCPs

Q

What steps are performed if LPI flow indicated?

A

1) align DH aux spray prior to sump recirc
2) If LPI flow > 2800 gpm with 2 pumps or > 3050 gpm with one pump then secure HPI, isolate CFTs, verify RCPs secured, secure EFW and close MFIVs/MSIVs.

Q

What actions are taken prior to BWST reaching 6 ft?

A

1) Throttle RB spray flow to 1050 - 1200 gpm to ensure NPSH to DH pumps
2) Open piggyback valves if HPI still in service

Q

What actions are taken during shift to RB sump suction (Att 1)?

A

1) Open RB sump outlets (If fails to open, secure associated HPI/DH/RB Spray pumps)
2) Close BWST outlets (If all LPI flow lost stop HPI/LPI and RB Spray pumps
3) If only one train LPI operating and power available to opposite train LPI suctions valves then align to opposite train RB sump suction via RCS suction line.

Q

What indicates RB sump blockage and what actions are taken?

A

(RB sump level dropping, Fluctuations in HPI/LPI/Spray flows and discharge pressures)

If blockage indicated

1) Reverify flow path
2) Throttle LPI flow to minimum flow (2 pumps - 2800 gpm, 1 pump - 3050 gpm)
3) If both RB spray trains operating and no evidence of containment breach then stop both spray pumps. If breach exists secure one RB spray pump.
