A

1) If A MFP in service and not maintaining properly, then Trip it and perform Loss of SG feed.
2) Check systems started appropriately (ICW, CRD Pump, MFP lube oil pumps, P-6A Running)

1) Perform loss of ICW in conjunction
2) If all ICW pumps off, then trip rx and trip RCPs due to loss of motor cooling, RT-5.
3) If B MFP not maintaining flows, then open crosstie valve and trip B MFP

What are major actions for loss of B-5?

A

1) Select B56 to B-6
2) Ensure EDG running with output breaker closed, A-309 open, service water pump running on A3
3) If OP HPI pump tripped isolate Letdown
4) If B5 cannot be energized, Place EDG output breaker in PTL, Diesel in lockout, close SW crossties to maximize flow to operable diesel
5) Begin plant shutdown and cooldown
6) If B5 was re-energized, shutdown EDG and restart OP HPI pump
7) Realign HPI

What are major actions for Loss of B-6?

A

1) Select B55/56 to B-5
2) Isolate Letdown
3) Throttle open TGLO and Gen H2 valve (Loss of Y02 causes them to close)
4) Close CV-1206
5) Ensure #2 EDG running and output breaker closed
6) If B6 cannot be energized, secure EDG and begin plant SD/CD.
7) If B6, not energized then use ES stby HPI Pump with suction from BWST as necessary for RCS makeup.

3) If OP HPI pump stopped, trip all RCPs and perform loss of reactor coolant makeup, RT-5
4) TRIP B MFP
5) DG running supplying bus with SW Pump running
6) Perform Loss ICW in conjunction (down to 1 ICW pump)
7) Address MISC systems (Condenser vacuum, IA, chill water, SF Cooling, condensate transfer)
8) If Y02 de-energized manually open TGLO and GEN H2 valves
