what are 2 potential meanings of evidence based practice
- use of research as ONE of many forms of evidence
- process by which poor/ineffective pracitces are weeded out and better practice is identified and implemented
define evidence based practice
using best available evidence, clinical expertise and patient values to make informed decisions about healthcare
forms of non-research forms of evidence: peer review, clinical audit (experimental data technically), benchmarking, established clinical expertise, tradition (e.g it has always been done this way), experience, policy and guidelines
what is benchmarking as a form of evidence
- setting/establishing of a standard (benchmark) by community of practitioners and followed by them
anecdotal evidence means non-research evidence
thus literature can be categorised into research and anecdotal
studies (not of standard of scientific criteria) can use cheap methodologies than fail to give best form of control and randomisation thus suffers from effect of bias or more variables unaccounted for.
debatable if adds to knowledge and evidence base.
too much reliance on intuition can work against principles of evidence-based practice
e.g of intuition is nurse wants to ask for much specification of patient condition etc but it’ll take to long so she asks for main symptom and intuitively adjusts the room, equipment etc for what she suspects will be needed
unpublished endeavour
- e.g practitioner or benchmark group uses research to find somehthing new but the work isnt accessible to wider audience of practitioners and cant benefit for that research. despite local intiative 9the practitioner) managing to base practice on evidence, it is their responsibility and duty to ensure that knowledge is passed on
research is important for quality improvement
research should be well designed and good quality
research should be justifiable in terms of benefits, safe and cause no harm
research must have informed consent
participants can withdraw at any time
info about patients must be confidential
basically, research generates the knowledge and EBP applies it
