RLQ pain, diarrhoea, mouth ulcer
Crohn’s disease
LLQ pain, bloody diarrhoea with mucus, mouth ulcer
Ulcerative colitis
Abdominal pain relieved by defecation (chronic), diarrhoea/constipation
IBS
Diarrhoea, abdominal pain, foul smelling stool that is pale and difficult to flush
Coeliac disease
- malnutrition+weight loss and dermatitis herpetiformis
Peri-umbilical pain that localises at the RIF, fever, n+v
Appendicitis
LLQ pain, constipation, rectal bleeding
Diverticulitis
RF - Marfan’s, Ehler Danlos, low dietary fibre
Cramping and intermittent abdominal pain, bilious vomiting and constipation
Small bowel obstruction
(May have had recent surgery or crohn’s disease)
(more frequent vomiting than LBO)
Cramping, constant abdominal pain, feculuent vomiting then constipation
Large bowel obstruction
(weight loss suggests malignancy, volvulus)
Cramping abdominal pain with diarrhoea, n+v, fever
Infective gastroenteritis
Epigastric pain, n+v (no other symptoms)
Gastritis
Retrosternal burning chest pain that is worse with spicy foods and on lying down
Gastro-oesophageal reflux disease
- nocturnal cough
Epigastric pain that is worse during a meal and better when not eating
Gastric peptic ulcer
- Possible weight loss
Epigastric pain that is worse in between meals
Duodenal peptic ulcer
Non progressive dysphagia, regurgitation, retrosternal chest pain
Achalasia
Progressive dysphagia, weight loss, malaena
Oesophageal cancer
Adenocarcinoma - barrett’s
Squamous cell carcinoma - smoking and alcohol
Epigastric pain, dysphagia, weight loss
Gastric cancer
Hematemesis, severe vomiting and retching
Mallory weiss tear (alcohol binge)
Hematemesis, history of liver disease
Oesophageal variceal bleed
