HPA is suppressed with supra physiologic doses; if a patient is on GCs prior (<1yr) to surgery, need to supplement because they cannot mount the typical cortisol response
mifepristone RU-486 moa
binds with high affinity to GC receptor complex in the nucleus (antagonist)
mifepristone RU-486 use
only for patients with ectopic ACTH secretion or adrenal carcinoma who have failed to respond to other therapeutics
mifepristone RU-486 adverse effects
vaginal bleeding, abdominal pain, GI (diarrhea), headache
spironolactone moa
aldosterone receptor antagonist + weak antagonism of androgen receptors
eplerenone
aldosterone receptor antagonist
spironolactone and eplerenone adverse effects
hyperkalemia and gynecomastia (spironolactone only)
fludrocortisone adverse effects
salt and fluid retention, CHF, sign and symptoms of GC excess
